Anyone know of a longer gear shifter?
I have a heck of a time fitting my boot under the lever... Could do with it about an inch longer!

Be easy enough to modify a lever to be longer. But you will get a lot more travel so your toe will have to lift/push far more. In the old days I used to have Kawa triples and there was quite a complex series of levers and joints - but this was before good rose joints and they used to wear. Sometimes the length of travel on these bikes to change gear was so bad you couldn't do it with a toe and ankle movement. you had to lift the whole foot off the footpeg!
Why don't you get some smaller boots?

You need some threaded bar (cant remeber the grade) a few nuts and one of these http://www.ebay.co.uk/itm/M5-Stainless- ... 3f162f9a0b
Bit of hacksaw action and your done.
Essentially its what you get in the adjustable footrest kit.

You could always hit up a metal works shop and get one custom made. I mean I don't think it cost that much. Plus you can get it built to your spec.
